V is F#, the natural form would be F#m...
iv (natural) would be Em... (but!)
Sacred music moved around a lot between natural
and harmonic minors, and even resolved the final
i chord to a I (the Picardy 3rd). The pope said
it was ok. So the iv V and v V exist a lot together
in the same pieces. As well as the i I (or V of IV).

the IV of B minor KEY, not IV of a B chord.

The 6/5 configurartion suggests that it is an inversion, and in this
case the chord is B7 with D# in the bass. And a B chord is the V of
IV (which is E as you say).
The 6 does not indicate a sixth in the chord but an interval above the
bass note - as you correctly identified.
