We solved this by modifying our client app to log the email,
clientEmail, operations and quota headers after each SOAP request. It
just logs it into a CSV file which can be easily post-processed to
attribute costs to clients.
An update to our AdWords API Toolkit which incorporates these changes
will appear soon (sooner if someone requests it from me).
